Researchers created an anatomically accurate sagittal-plane model of the entire spine (C2 to S1) based on real full-spine X-rays of healthy adults. They then simulated what happens when vertebral bodies lose strength and stiffness due to osteoporosis while normal body-weight forces (gravity + postural muscles) are applied.

Key findings that match what we see clinically:

  • In the most severe osteopenic model, thoracic kyphosis jumped from a normal ~40° to 81.6° — a 41.7° increase.
  • Anterior wedge-like fractures appeared exactly where they’re most common in real patients: T7 and T8.
  • Spinal height (C2–S1) dropped by 25.2% and total body height by 8.6%.
  • The upper spine shifted forward by 15.1 cm, creating a vicious cycle: forward translation increases compressive loads on the thoracolumbar region, which accelerates more wedging and deformity.

The study’s conclusion was clear: Postural stresses, not trauma, are often enough to initiate osteoporotic kyphosis in weakened vertebrae. Once the upper body shifts forward, the problem snowballs.
